Message from the ADA President

The president of the American Diabetes Association in his presidential address gave a sobering speech about investing in prevention in order to save on complications (of diabetes, of course). By doing so, he claimed, there would be 5 million fewer heart attacks, 600,000 fewer strokes, 1.2 million fewer cases of renal failure, and 1.8 million fewer premature deaths. To reach such a goal, 80% of patients must meet the following guidelines: 1) HbA1c should be less than 7%, 2) blood pressure should be less than 130/80, 3) LDL (the bad cholesterol) should be less than 100, 4) HDL (the good cholesterol) should be greater than 40 in men, and 50 in women, and 5) everyone should take a daily baby aspirin. Dr. Robert A. Rizza even went further to suggest that every type 2 diabetic should take a daily generic “polypill” consisting of 1,000 mg metformin, 75 mg aspirin, 40 mg statin, and 10 mg of an ACE inhibitor. Such a generic pill would cost about $100 per year while saving about $400 per year. Short of finding a cure, the above advice is very reasonable and should be adopted by the health care industry.

Once-Weekly Byetta

Some of you who are happily injecting Byetta twice daily, maybe glad to hear that a long-acting weekly preparation is on its way. It may not be on the market until 2008, but its coming.

Galvus and Januvia

These are two new drugs for type 2 diabetics which will probably shortly become approved by the FDA. Both belong to a new class which is called DPP-4 inhibitors, and are manufactured by Novartis and Merck respectively. Both inhibit the enzyme DPP-4 which is responsible for metabolizing (destroying) a gastrointestinal hormone called glucagon-like peptide (GLP). GLP is made by intestinal-lining cells when they are exposed to carbohydrates in the diet. GLP travels to pancreatic beta cells where it stimulates insulin production, and thus augments the response to ingested food. Both drugs seem to have comparable glucose-lowering effect when compared to other oral agents, and hold even greater promise when used in combination with other oral agents. These drugs constitute the SIXTH category within the oral agents, without any end in site!
